Extended Health Patients are required to pay for services provided in advance and submit our invoice and their claim form directly to their Insurance for re-imbursement.
The cost of Orthotics, Orthopedic Shoes, Cervical Pillows, Lumbar supports and other assistive devices as defined, must be paid for in full prior to them being ordered. All sales are final after the supplies are ordered.
In order to serve you and other patients better, we require a 24 hour prior notice to cancel your scheduled appointment. A missed appointment fee of $20 will apply to patients who fail to cancel their scheduled appointment without 24 hour prior notice.
We accept cash, visa, master card, debit, peronal cheques as well as extended health coverage, WSIB and auto insurance benefits. The fees are subject to change without notice. Our services are not covered by OHIP.
